Job Shadowing - Provider

Thank you for your interest in job shadowing at HSHS. Please complete the steps below to begin the application process.

Please note: If you are an HSHS colleague, please fill out the below documentation for any shadow/clinical at HSHS Physician Enterprise. Otherwise, please contact the department you are interested in shadowing to make job shadowing arrangements.

  1. Review the HSHS Job Shadow Policy and fill out the 2 page form at the end of the policy. Complete and submit the form with this application.  (see “Upload HSHS Job Shadow Policy Form” below)
  2. Fill out Attestation Form:  Download, complete and submit the HSHS Job Shadow Immunization Attestation with this application (see “Upload HSHS Job Shadow Immunization Attestation” in the form below). 
  3. Fill out the TB Screening Checklist if documentation for TB test or chest x-ray within one year are not available.
  4. Once all information is processed, a representative will contact you.
