Regular, progressive exercise and disease management can be extremely beneficial in recovery from illness or injury, in managing chronic health conditions, and as a goal in improving overall fitness and conditioning levels.

The HSHS St. Nicholas Hospital Cardiac Rehabilitation Department encompass a variety of therapeutic exercise programs for patients with cardiovascular disease.  Our program is for patients immediately following a hospital inpatient discharge from a cardiovascular event.

Cardiac Rehabilitation

Cardiac rehabilitation is a specialty service designed to assist individuals to recover fully following a heart attack or other heart problem. Your physician will determine when an outpatient program is right for you and will submit a referral to cardiac rehab. Cardiac rehab offers monitored exercise sessions and education needed to return to a heart healthy lifestyle.

Tertiary Care

Sometimes certain health conditions, such as those requiring open heart surgery are best treated at tertiary care facilities. Tertiary care facilities offer healthcare services provided by specialized practitioners and which require highly sophisticated technologies and equipment. Admission to a tertiary care facility usually requires a referral from a physician.

HSHS St. Nicholas Hospital and Physicians' Health Network partner with tertiary care facilities in our region to best meet the needs of patients and their families. These facilities include but may not be limited to the following.

The tertiary care provider(s) available to you are determined by your insurance plan. Check with your health care insurance provider or your employer for information about the tertiary care facilities included in your health plan.
