Social work services

Social workers in HSHS St. John's Children's Hospital provide a wide variety of assistance to patients and families through resource referrals, care coordination with community agencies and discharge planning.

Counseling and advocacy services are also available as needed.

Services include:

  • Resource connections
  • Crisis management
  • Care coordination
  • Family support

At the bedside and behind the scenes, social workers work with the medical team to coordinate care of the patient and family to ensure a safe discharge from the hospital setting.
