Medical record requests

To request a copy of your medical records from our Health Information Department , download and complete the Medical Records Release Form. Once completed, attach it to the online form below, or mail to: Health Information Department, 1 St Elizabeth's Blvd O'Fallon, Illinois 62269. Call 618-234-2120, ext. 11742 for more information. 

STEP 1
Download and complete the Medical Records Release Form.

STEP 2
Then complete the authorized request through the following options:

  • Call or visit our Health Information Department at 618-234-2120, ext. 11742. We are open from 8 a.m. to 4:30 p.m., Monday through Friday. The Health Information Release of Medical Records office is located just inside the Outpatient Entrance on the first floor near the Registration desk. A photo identification will be requested to release copies of medical records.

  • Fax a request or authorization form to 618-222-4777.
