Prairie Heart Institute allows Holy Family to transition from treating episodic care to having “one call” direct access to advanced heart care services with Prairie Heart Institute 24/7.  

If you need further advanced heart care services, we provide direct access to the Cardiac Cath Lab at HSHS St. Elizabeth's Hospital or HSHS St. John's Hospital, where the Prairie Heart surgeons perform nationally-recognized, life-saving procedures.

Cardiac testing procedures (EKG, Echo, Stress Test, Cardioversion, and more) are available on-site at Holy Family Hospital in the Specialty Clinics.

Cardiac Rehabilitation

Working closely with our medical director, physicians, and staff provides a multidisciplinary approach of education and monitored exercise to enhance your quality of life. Cardiac rehab will return you to your daily activities while improving your health.

About Cardiac Rehabilitation

The Center for Cardiac Rehab & Wellness at Holy Family Hospital includes several programs specifically designed to help reduce risk factors and improve overall health and wellness. The programs target specific health issues appropriate to each program. Areas of focus include cardiac, pulmonary, vascular and the management of chronic diseases such as diabetes, heart failure, cancer and COPD. 

Our programs include: 
  • Cardiac Rehabilitation
  • Pulmonary Rehabilitation
  • Peripheral Arterial Disease (PAD) Supervised Exercise Therapy
Cardiac rehab is a program of up to 36 sessions based on an individualized and personalized treatment plan to strengthen the heart and reduce risk factors after a cardiac event such as 
  • Heart attack
  • Angina
  • Coronary Artery Angioplasty or Stents
  • Open Heart Surgery such as Coronary Artery Bypass or Heart Valve Surgery
  • Heart Failure
  • Heart Transplantation
It is generally covered by most health insurance companies. Recent scientific studies have shown that people who complete a cardiac rehabilitation program can increase their life expectancy by up to five years.

Cardiac rehab offers
  • Education and support to make healthy lifestyle changes such as healthy eating, maintaining a healthy weight, avoiding tobacco and environmental smoke and managing stress
  • Beginning and maintaining a personalized exercise plan that works for you
  • Monitoring and assistance to better control blood pressure, lipids / cholesterol and diabetes 
Pulmonary Rehab is a program of up to 36 sessions based on an individualized and personalized treatment plan to improve symptoms of shortness of breath associated with various lung diseases, including
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Pulmonary Fibrosis
  • Interstitial Lung Disease
  • Lung Volume Reduction Surgery 
  • Before and after Lung Transplantation
It is generally covered by most insurance companies and assists participants to live better with their lung condition. A Pulmonary Function Test within the last year is required by most insurances prior to starting the program.

Pulmonary rehab offers
  • Monitored and supervised physician-prescribed exercise to improve endurance and muscle strength to help minimize symptoms of shortness of breath
  • Psychosocial support to manage living with lung disease and improve quality of life
  • Education focused on breathing training, strategies for dealing with breathing problems and symptoms, diet and nutrition and medications
The Supervised Exercise Therapy for Peripheral Arterial Disease (SET for PAD) is a program to improve circulation and decrease symptoms of pain with walking associated with peripheral arterial disease. 

Your physician must have diagnosed symptomatic PAD and refer you to the program. He may perform testing to verify the PAD, including non-invasive ABI testing.

The SET program offers
  • Monitored and supervised exercise to improve circulation
  • Assessment and plan for exercise to improve functional ability and decrease symptoms
  • 36 sessions, 1-hour per session
  • Coverage by most insurances
